Risk Management

These are the risk and insurance forms that need to be completed when we organise an event that involves the public.

The first two forms need to be filled out for each event.

1 Public & Products Liability form

2 Record of Assessment  clicking on his link will download the word document

3 Rotary District 9520 - Integrated Risk Matrix   This form  is used when completing part of form 2. 

The organiser of the event can work with me to complete the forms. 
The forms then need to be signed as approved by the Club President and then I send them to the District Insurance Officer, who forwards them to the District's insurance company.

4 Rotary Club of Campbelltown Incident and Hazard Report  to be used to report any hazard or incident.
